NHS England South West’s regional Digital Neighbourhood Teams programme enabled clinicians to identify patients that could benefit from early support and interventions, reducing their likelihood of going to hospital. It was essential that there was a joined up approach between health, social care and community providers building a Team of Teams. John’s experience and understanding of the different groups, the specifics of their approaches and particular language was a requisite element in being able to drive the transformation.
Holding a diverse portfolio on the programme board, his people orientated approach supported those within the Programme Team and in the region wide delivery-sites to establish and collaborate on the necessary tasks.

I worked alongside John throughout the Kit4Care programme, which he conceived, chaired and led from a small test of merit through to a scaled, funded proof of concept connecting primary and social care. I was part of the core team that helped John bring the ‘Shaping Future Care’ conference to fruition.
